The IT department's offboarding process receives a notification to retrieve the equipment(s) assigned. Please have the responsible person collect the items below.
☐ Laptop Asset tag:____________________(silver sticker found below the laptop screen)
☐ Power adapter (not the docking station) It should be a Dell 65-watt travel brick.
☐ Portable Monitor (some liaisons will have this extra screen to work with their laptops) It should only have a USB C cable.
* Please report any damage or missing items to the IT department. The hospital could incur a cost for any missing items.
Employees who work from home may have the following equipment. Please retrieve these items.
☐ Monitor(s), Dell. Each monitor should have a power cord and video cable.
☐ Docking station
☐ Docking station Power Supply (180-watt power supply) This should not be the travel power supply which is smaller.
☐ Keyboard and Mouse (this is up to the hospital to retrieve)
Retrieval person notes:
! Laptops are disabled when an employee is offboarded. This helps prevent employees or departments from borrowing without permission. Please ensure the assigned equipment stays with the department. The equipment was preauthorized for the departing employee and position. If the asset needs to be transferred, please create an incident.

This checklist can be kept with the equipment for your reference. The IT department does not need this document. Please reply to the offboarding ticket with the Asset tag and/or any missing items.
